Physicians for Kids Discovery SeriesOrder your Discovery Series shows online! It's as easy as 1-2-3.

  1. Simply click here to open the order form.
  2. Type your information into the PDF form. Information with a red box is required.
  3. Click the submit button at the bottom of the form. The form will be emailed to the Physicians for Kids Discovery Series.

If you would like to complete the form and mail or fax it back to us, please use the Physicians for Kids Discovery Series printable form.

Please note, after September 15, 2010, all Discovery orders must be paid in full at time of order and can only be placed by fax or phone.

Any changes to your order must be received at least two weeks prior to the performance.
